Abstract
Objective: To understand the interference of HIV testing performed at hospital admission for childbirth on breastfeeding within the first hour of life. Method: A qualitative, exploratory-descriptive study conducted between December 2024 and June 2025, using semi-structured interviews at a maternity hospital in the Baixada Litorânea region of Rio de Janeiro, Brazil. A total of 35 participants were included: 20 healthcare professionals (five nursing technicians, 12 nurses, and three physicians) and 15 family members (14 postpartum women and one newborn’s aunt). Data were processed using the software Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires (IRAMUTEQ) and subjected to Thematic Analysis. Results: HIV testing at hospital admission for childbirth directly interferes with breastfeeding within the first hour of life, delaying the initiation of this practice. Although it constitutes an institutional routine, its implementation varies according to the professionals’ practices and the type of test performed (rapid or laboratory), and it is affected by material shortages. This dynamic compromises breastfeeding, often resulting in the use of formula. Conclusion: Care workflows must be reconsidered so that HIV testing at hospital admission for childbirth does not hinder breastfeeding within the first hour of life, allowing both practices to occur in an integrated, safe, and timely manner.
